The neuromodulator landscape has changed more in the last few years than it did in the previous two decades. For most of that time, patients had one choice — Botox®. Then came Dysport®, then Xeomin®, then Daxxify®, then Letybo®. Relfydess® by Galderma represents something different — not just another botulinum toxin, but a fundamentally different approach to how neuromodulators are made, stored, and delivered.
I want to walk you through what Relfydess® actually is, how it compares to the other options, and what it means for patients who are either already using neuromodulators or considering them for the first time.
WHAT MAKES RELFYDESS® DIFFERENT
Every other neuromodulator on the market arrives at your provider’s office as a freeze-dried powder. Before it can be injected, it must be reconstituted — mixed with saline to create a liquid solution. This step introduces variability. Different providers may use slightly different dilution volumes, different saline types, or different mixing techniques. The result is that your Botox® experience at one practice may not feel identical to your Botox® experience at another, even at the same unit count.
Relfydess® eliminates this variable entirely. It is the first and only neuromodulator that arrives as a ready-to-use liquid. No reconstitution. No mixing. No dilution variability. Every vial contains a fixed concentration of 100 units per milliliter, shelf-stable in liquid form for over 18 months. What this means in practical terms is that the product your provider draws into the syringe is the same product every single time — batch to batch, vial to vial, visit to visit.
This may sound like a manufacturing detail, but it has real clinical implications. Consistency in concentration means consistency in dosing, which means consistency in results. For patients who value predictability in their outcomes — and that should be every patient — this matters.
THE SCIENCE BEHIND THE FORMULATION
Relfydess® is produced using what Galderma calls PEARL technology — Precipitation, Extraction, and Activity-preserving Refined Liquid. The process starts with a proprietary strain of Clostridium botulinum type A1 and uses multi-step purification including diafiltration, ion-exchange chromatography, and size-exclusion chromatography to isolate the active botulinum toxin molecule.
Two things are worth highlighting here. First, Relfydess® is complex-free — it contains no accessory proteins. To understand why this matters, consider that Botox® is delivered as a 900-kilodalton protein complex, meaning the active toxin molecule is surrounded by a cluster of non-therapeutic proteins. These accessory proteins don’t contribute to the wrinkle-relaxing effect. They are, in a sense, biological packaging material. The concern — supported by some immunologic research — is that these extra proteins may increase the likelihood of antibody formation over time, potentially leading to diminished response with repeated treatments. Complex-free formulations like Relfydess® remove that variable.
Second, the PEARL process keeps the botulinum toxin in its native liquid conformation throughout manufacturing. Freeze-drying — the process used for every other toxin — subjects the molecule to physical stress that can alter its three-dimensional structure. Published data shows that Relfydess® contains more active botulinum toxin per on-label dose (0.27 nanograms) compared to Botox® (0.18 nanograms), with higher measured enzyme activity. In simple terms: the molecule arrives intact, active, and ready to work.
HOW RELFYDESS® PERFORMS
The clinical evidence behind Relfydess® comes from the READY program — four Phase III clinical trials enrolling more than 1,900 participants.
Onset is where Relfydess® makes its first impression. In the Phase IIIb RELAX trial, 39% of patients reported visible improvement in frown lines at Day 1. That is fast. Most patients using Botox® expect to wait three to seven days. Dysport® tends to onset in two to three days. Daxxify® is typically one to two days. Relfydess® appears to be in the fastest tier, with a meaningful percentage of patients seeing results within 24 hours.
Efficacy at peak effect is strong. In the READY-1 trial for glabellar lines (the vertical frown lines between the eyebrows), 82.9% of patients achieved a composite two-grade or greater improvement at one month, compared to 0% with placebo. By investigator assessment, 96.3% of treated patients had none-to-mild frown lines at the one-month mark.
Duration is the number patients always ask about, and Relfydess® delivers a compelling answer. In the RELAX trial, the median time to return to baseline was 24 weeks — approximately six months. At the six-month mark, 69% of subjects were still satisfied with their appearance. In the READY-1 trial, 58% of patients maintained at least a one-grade improvement in their frown lines at Month 6. This puts Relfydess® in the longer-duration category alongside Daxxify®, and well ahead of the three-to-four-month duration typical of Botox®, Dysport®, and Letybo®.
The safety data across all four READY trials was reassuring. Treatment-related adverse events occurred in only 3.6% of participants in the pivotal trial. The most common side effects were mild injection site reactions and headache — the same profile we see across all neuromodulators. Importantly, the long-term READY-4 study showed no new safety signals across four consecutive treatment cycles, with adverse event rates remaining low and consistent over time.
HOW IT COMPARES TO WHAT’S ALREADY AVAILABLE
I find that patients benefit from understanding the full landscape, not just the most recent product. Here is how I think about the current neuromodulator options:
Botox® (onabotulinumtoxinA) remains the most widely recognized name in the category and carries the longest track record — over two decades of clinical use. It is effective, well-understood, and versatile across cosmetic and therapeutic indications. Its limitations are its moderate duration (three to four months for most patients), the need for reconstitution, and the presence of complexing proteins. For many patients, Botox® remains an excellent choice. But it is no longer the only excellent choice.
Dysport® (abobotulinumtoxinA), also manufactured by Galderma, has a slightly faster onset than Botox® and a diffusion profile that some providers prefer for broader areas like the forehead. It uses a different unit scale — Dysport® units are not equivalent to Botox® units — which can create confusion. Its duration is comparable to Botox® at three to four months.
Daxxify® (daxibotulinumtoxinA) was the first major innovation in durability, with clinical data supporting six to nine months of effect for many patients. It uses a proprietary peptide stabilizer instead of human serum albumin, and it is also complex-free. Daxxify® changed the conversation around how long neuromodulators could last, and it remains the benchmark for extended duration. We use Daxxify® frequently at 360 Plastic Surgery and patients who have tried it consistently appreciate the longer interval between treatments.
Letybo® (letibotulinumtoxinA) is a 150-kilodalton purified toxin with faster reported onset than Botox®. Its duration is in the traditional three-to-four-month range. Letybo® adds another option to the conventional-duration category but does not fundamentally change the performance envelope.
BonTE® (trenibotulinumtoxinE) represents something entirely different from every product listed above. While all other neuromodulators are derived from botulinum toxin serotype A, BonTE® is derived from serotype E — a fundamentally distinct molecule with different pharmacokinetics. The clinical significance is striking: onset in as little as six to eight hours with peak effect at approximately 24 hours, and a duration of two to four weeks. That is not a limitation — it is the point. BonTE® is designed for patients who want rapid, short-duration muscle relaxation — think of a patient who wants to look refreshed for a specific event but prefers not to commit to months of effect, or a first-time patient who wants to experience what a neuromodulator feels like before committing to a longer-acting product. It also opens therapeutic possibilities for conditions where shorter-duration paralysis is clinically preferable. BonTE® does not compete with Relfydess® or Daxxify® so much as it creates an entirely new category.
Relfydess® occupies a unique position in this landscape. It combines the longer duration profile (six months) seen with Daxxify® with the ready-to-use liquid formulation that no other product offers. Its rapid onset — Day 1 for a significant percentage of patients — is a practical advantage for patients who want to see results quickly, particularly those scheduling treatments ahead of events. The elimination of reconstitution variability is an advantage that compounds over time: every treatment is dosed from the same standardized concentration.
Where I see Relfydess® fitting most naturally is for patients who value consistency and duration — those who want a longer-lasting result and want to know that what they’re receiving is identical from one appointment to the next. It also has potential appeal for patients who have experienced diminished response to other toxins over time, as the complex-free liquid formulation may carry a lower risk of antibody-mediated resistance.
DOSING AND TREATMENT AREAS
Relfydess® is indicated for the treatment of moderate-to-severe glabellar lines (frown lines between the eyebrows) and lateral canthal lines (crow’s feet). In clinical trials, glabellar lines were treated with 50 units distributed across five injection sites, and crow’s feet were treated with 60 units distributed across six injection sites. Combined treatment of both areas used a total of 110 units.
An important note: units of Relfydess® are not interchangeable with units of any other botulinum toxin. Each manufacturer uses a proprietary potency assay, which means that 50 units of Relfydess® is not the same as 50 units of Botox® or 50 units of Dysport®. This is true across all neuromodulators and is one of the reasons it is critical to be treated by a provider who understands the specific pharmacology and dosing of the product they are using.
WHAT THIS MEANS FOR PATIENTS
The addition of Relfydess® to the neuromodulator market gives patients and providers something valuable: meaningful choice. Not every patient responds identically to every toxin. Some patients metabolize Botox® quickly and are back in three months; those patients may benefit from a longer-duration option like Daxxify® or Relfydess®. Some patients develop subtle resistance to one product and do well when switched to another. Some patients simply prefer the idea of a standardized liquid formulation over a reconstituted powder. And with BonTE® offering an ultra-short-duration option at the other end of the spectrum, we are approaching a future where neuromodulator treatment can be truly tailored — not just in dose and placement, but in how long the effect lasts.
At 360 Plastic Surgery, my philosophy has always been that the best outcomes come from matching the right treatment to the right patient. That requires understanding the full range of options — not just the most popular or the most marketed — and making individualized recommendations based on your anatomy, your goals, your treatment history, and your preferences.
If you are currently using a neuromodulator and happy with your results, there is no urgent reason to switch. If you are curious about longer duration, faster onset, or a formulation with less batch-to-batch variability, Relfydess® is a conversation worth having at your next appointment.
If you have never tried a neuromodulator and are considering your first treatment, the expanding menu of options can feel overwhelming. The most important decision is not which product to use — it is who is administering it. A board-certified plastic surgeon or a highly trained injector who understands facial anatomy, muscle dynamics, and the pharmacologic differences between products will deliver a superior result regardless of which toxin is in the syringe. The product matters. The hands holding it matter more.
BOTTOM LINE
Relfydess® is not simply “another Botox®.” It is a genuinely differentiated neuromodulator — the first liquid, ready-to-use formulation, backed by a robust clinical trial program demonstrating rapid onset, high efficacy, and duration that extends to six months for most patients. It enters a market that is finally offering patients the kind of choice and customization that I believe they deserve.
